Definitely Amnesia: The Dark Descent. The game oozes atmosphere and freaks you out long before any monsters actually appear.

Certain sections of the Dead Space games.  One that comes to mind is when you are being stalked by an unkillable necromorph in the 2nd game.  Any game that has something like that creeps me out.

The most a game has ever gotten under my skin was when I played Silent Hill 2 in the dark while home alone on Halloween. Coming out of my parents' basement into the empty and dark house was extremely unnerving.

Fatal Frame 2 Crimson Butterfly. Still haven't finished it after all this time, scared me into quitting.

Dead Space on the PS3. It wasn't scary but I didn't know much about the game and having things unexpectedly pop up, and having that uneasy tension throughout the game. There was always a sigh of relief when I made it to the train car because I knew the level was done.

I got really freaked out by Doom once, the original one on PC. I was wandering around a seemingly empty area looking out for Imps. The bleak atmosphere and the gloomy music started working on me, and I started getting the shakes. Sometimes it's when nothing is happening that you get scared the most.

Amnesia. Okay, it's only the first minutes and I haven't even encountered the first enemy, but if the game can already be creepy without even a monster, that is something amazing.

I'm going to say Silent Hill 2, and I chalk a lot of that up to the lack of hope in the game - you just know things are going to get worse and worse as you unravel the truth of why James has been called to Silent Hill.

Silent Hill when you are in the elementary school just creeped me out.  I was really spooked. But I never finished the game. so that part is the scariest part of a game I've ever played.  As for finished games.. I would go with Silent Hill 2. especially when you are in the old prison.  The sound effects just creeped me out.

I really don't play scary games because I have an over-active imagination to start with. That said, there's a spot where I jumped and squeeked at the end of the first episode of The Last Door.
